Request form (due April 15)
All payments should be made through this request form.
- For accompanying children under the age of 12, we have a discount for meals. Use this form for children instead.
- Change the file names appropriated and send it to LOC by e-mail <iachec2019loc at heal.phy.saitama-u.ac.jp> by April 15.
- One form should be filled for one receipt. You can arbitrary break your charges into reimbursed and non-reimbursed parts.
- Do NOT make reservations through the Shonan Village Center; otherwise you will be charged twice.
- All rooms are booked on May 18 (Sat) in the Shonan Village Center. Please find a hotel in near-by cities on May 18.
- We may have limited support for travel fees. If you request, please answer yes to the question in the registration form.
Payment (due May 15)
- The payment should be completed by May 15 by credit card.
- Online payment information will be sent by e-mail individually around May 7.
- The registration fee is lowered from 12,000 yen to 7,200 yen.
- The Shonan Village Center has a full responsibility in accounting. Do not pay anything to the LOC members locally. The receipts will be issued by the Shonan Village Center.
